What You'll Do7
- 1Evaluate patients during initial home visits and draft individualized plans of care within 48 hours
- 2Implement therapeutic exercises for functional strengthening, gait training, and balance using TherapySync EMR
- 3Schedule home visits across Santa Clara cities including San Jose, Sunnyvale, and Mountain View based on your availability
- 4Collaborate with nurses, occupational therapists, and supervisors to adjust care plans for complex geriatric cases
- 5Document patient progress after each visit in compliance with Medicare and Title 22 standards
- 6Coordinate with the care team on DME needs and refer to community resources where appropriate
- 7Drive between patient residences using a reliable vehicle with valid insurance and registration
Requirements6
- 12+ years of physical therapy experience, including home health, outpatient, or skilled nursing settings
- 2Valid California Physical Therapist license
- 3CPR or BLS certification
- 4Driver's license and auto insurance; TB test within the past year
- 5Professional liability insurance and business license if applicable
- 6Ability to manage a caseload in Santa Clara County with reliable transportation
