What You'll Do11
- 1Deliver patient-centered care by evaluating functional status and designing individualized treatment plans
- 2Implement hands-on therapy to improve mobility strength and independence
- 3Educate patients and caregivers on treatment protocols and safety strategies
- 4Monitor progress and adjust interventions to achieve recovery goals
- 5Coordinate safe care transitions and participate in discharge planning
- 6Identify patient needs and collaborate with interdisciplinary teams
- 7Lead and supervise Physical Therapist Assistants following state regulations
- 8Document patient records accurately in the EMR
- 9Participate in case conferences and team meetings
- 10Provide guidance to caregivers and support staff
- 11Drive outcomes through proactive assessment and adaptive care strategies
Requirements9
- 1Graduate of an accredited Physical Therapy program with DPT or Master's degree
- 2Current physical therapy license in the state of practice
- 3Minimum 1-2 years home health experience preferred
- 4Strong clinical assessment and critical thinking abilities
- 5Excellent communication and patient education skills
- 6Valid CPR certification
- 7Reliable driver’s license with transportation and auto insurance
- 85+ years building ETL pipelines with Spark and Airflow
- 9Bold certifications: CPC Card BCBA AWS Certified
