What You'll Do11
- 1Manage fast-paced independent schedules while delivering compassionate care
- 2Conduct OASIS assessments to evaluate patient needs and determine service appropriateness
- 3Develop and modify care plans in collaboration with patients caregivers and representatives
- 4Document clinical notes and process orders promptly and accurately
- 5Supervise PTAs and aides to ensure consistent care delivery
- 6Apply infection control protocols during patient interactions
- 7Provide patient education on mobility techniques and equipment usage
- 8Coordinate with physicians to implement treatment plans
- 9Maintain compliance with agency and licensing requirements
- 10Adapt treatment approaches for neuromuscular neurological and orthopedic conditions
- 11Communicate updates to healthcare providers regarding patient progress
Requirements5
- 1Minimum one year of home health experience with demonstrated ability to manage independent schedules
- 2Proficiency with Homecare Homebase EMR system
- 3Strong clinical assessment skills and experience with geriatric populations
- 4Excellent communication and interpersonal abilities
- 5Ability to work autonomously while supporting team objectives
Salary Insight
$104 - $312k per year
