Home Health Physical Therapist Assistant - Austin, TX
Overview
You will own physical therapy delivery for geriatric patients across the San Marcos area, reporting directly to the Director of Clinical Services. You will collaborate with a team of PTs, PTAs, and home health aides in a company recognized as Best in Home Health and Best in Home Care by the Senior Resource Guide. What You'll Do9
- 1Conduct OASIS assessments to identify patient needs and determine appropriate home care plans.
- 2Develop and evaluate the plan of care with patients, caregivers, and physicians.
- 3Document clinical notes, phone orders, and records in Homecare Homebase EMR.
- 4Revise the treatment plan based on ongoing assessments and patient progress.
- 5Supervise PTAs and home health aides on patient care tasks.
- 6Deliver treatments for neuromuscular, neurological, and orthopedic disorders.
- 7Communicate with physicians and other care providers to coordinate the plan.
- 8Educate patients and families on transfer techniques and safe equipment use like walkers and wheelchairs.
- 9Apply infection control and standard precautions in all patient activities.
Requirements6
- 1Current PTA license in Texas.
- 21+ years of home health experience preferred.
- 3Proficiency or familiarity with Homecare Homebase EMR is a plus.
- 4Strong clinical assessment skills for geriatric patients.
- 5Excellent communication and interpersonal abilities.
- 6Ability to work independently and manage a flexible schedule.
