Physical Therapist, Post-Acute Care & Rehabilitation
Overview
You will own the full rehabilitation cycle for Physical Therapy patients at a post-acute care center in Santa Clarita. You will evaluate residents, design individualized treatment plans, and deliver interventions that restore strength, mobility, and independence. You will partner with an interdisciplinary team of Occupational Therapy, Speech Therapy, and nursing staff to ensure seamless continuity from hospital to home. This role stands out because you will directly shape recovery outcomes for a diverse post-acute population, with new graduates actively welcomed.
What You'll Do8
- 1Evaluate residents on admission and during ongoing care to identify functional limitations and establish measurable goals
- 2Develop and implement individualized Physical Therapy plans of care based on clinical findings and patient goals
- 3Deliver skilled interventions including therapeutic exercise, balance training, transfer training, gait training, and mobility training
- 4Monitor resident progress and adjust treatment plans as clinical responses change
- 5Collaborate with physicians, nursing, Occupational Therapy, and Speech Therapy to coordinate care
- 6Communicate with residents, families, and caregivers to support engagement and adherence
- 7Complete accurate and timely clinical documentation compliant with regulatory standards
- 8Participate in discharge planning, recommending safe transitions to home or lower levels of care
Requirements8
- 1Current Physical Therapy license in the State of California
- 2Graduate of an accredited Physical Therapy program
- 3New graduates welcome to apply
- 4Skilled nursing or post-acute care experience preferred but not required
