Physical Therapist - Geriatric Rehab & Wound Care
Overview
You will own the full therapy cycle for geriatric patients, from initial evaluation to discharge, covering physical functioning disorders and wound care at Golden Acres' post-acute facility. You'll work with a dedicated rehab team using EPIC and Net Health to plan and deliver interventions. New grads and experienced therapists alike will find structured Clinical Development Pathways and Unlimited CEUs. What You'll Do6
- 1Screen and evaluate patients with physical functioning disorders and wound care, then write care plans that meet CMS and APTA standards.
- 2Deliver skilled interventions including manual therapy, therapeutic exercises, and wound care, using TheraBand and Game Ready.
- 3Administer modalities like ultrasound, electrical stimulation, and traction per the rehab plan.
- 4Counsel patients and families on physical disorders and home exercise programs.
- 5Document progress in Net Health and generate discharge summaries per regulatory requirements.
- 6Coordinate with nurses and physicians to adjust treatment plans based on patient response.
Requirements6
- 1Active PT license in Texas required.
- 2New Grads welcome; all experience levels considered.
- 3DPT or MPT degree from an accredited program.
- 4BLS or CPR certification required by start date.
- 51+ years experience in geriatric or post-acute rehab preferred but not mandatory.
- 6Ability to perform physical tasks like lifting patients and standing for long shifts.
