Physical Therapist, Home Health Per Diem
Overview
You will deliver rehabilitation services to home health patients across Richmond and nearby cities, owning the full care cycle from evaluation to discharge. You set your own schedule and income with per-visit rates up to $120. You work with a fast-growing healthcare tech company, supported by a care coordinator and a mobile app for job finding and time tracking. This role offers weekly pay, transparent rates before accepting visits, and a simplified documentation process.
What You'll Do10
- 1Evaluate patients' needs, design a care plan, and begin implementation within your first 90 days, using POC documentation standards.
- 2Communicate with the patient care team and adjust the care plan as needed, collaborating with physicians and other disciplines.
- 3Reach out for community services to assist patients as needed, building a network of local resources.
- 4Perform physical treatments with patients as per designed care plans, ensuring proper technique and safety.
- 5Review notes and charts of PTAs, sign off on their documentation, and communicate with them regularly to supervise patient progress.
- 6Collaborate with physicians, agency staff, and contract services to coordinate and implement the Plan of Care.
- 7Document direct patient services and coordination with the physician, ensuring completeness and compliance with home health standards.
- 8Review caseload charts documentation at least every 60 days to ensure compliance and patient progress.
- 9Assess patient progress and effectiveness of care delivery, adjusting goals as needed.
- 10Plan appropriate discharge and coordinate with the care team to ensure a smooth transition.
Requirements8
- 1Current Physical Therapist license in the State of CA.
- 2CPR certification and a negative TB test.
- 31+ years of home health experience.
- 4Valid driver's license and reliable transportation to travel to patients' residences.
- 5Ability to stoop, bend, and lift and transfer patients.
- 6Willingness to cover Richmond and nearby cities.
- 7Desired: background in public health, home care, or hospital setting.
- 8Desired: bilingual skills to communicate with diverse patient populations.
