What You'll Do8
- 1Make home visits to clients in designated geographic territories
- 2Perform diagnostic tests and measurements including mobility range of joints and strength assessments
- 3Implement integrative protective interventions using evidence-based dressings
- 4Develop and modify individualized care plans featuring manual therapy and gait training
- 5Continuously evaluate and update PT care plans in collaboration with nursing staff
- 6Educate clients and families on rehabilitative practices to enhance independence
- 7Document client observations and interventions using advanced digital tools
- 8Maintain accurate clinical records and ensure compliance with healthcare standards
Requirements5
- 1Current VA Physical Therapist license holder
- 2Graduated from APTA or CAMPEP accredited program
- 3Demonstrated ability to work autonomously with strong time management skills
- 4Excellent communication and problem-solving capabilities
- 5Proficiency with electronic medical records systems
