What You'll Do7
- 1Perform home visits across designated territories
- 2Conduct comprehensive PT evaluations assessing mobility strength balance gait transfers endurance and safety risks
- 3Develop individualized treatment plans to enhance patient function and independence
- 4Deliver therapeutic exercises gait training and balance training using evidence-based methods
- 5Collaborate with nurses and clinicians as part of a multidisciplinary team
- 6Educate patients and caregivers on exercises safety and disease management
- 7Document patient care using a user-friendly electronic medical record system
Requirements5
- 1Active and current NJ PT license
- 2Home healthcare experience preferred
- 3Ability to travel to assigned cases
- 4Calm reassuring presence active listening skills and strong communication abilities
- 5Commitment to delivering compassionate patient-centered care
Salary Insight
$125 - $146k per year
