Overview

Own care coordination for Group Home and HCBS programs at scale within a fast-paced, in-person role on-site in Phoenix. Collaborate with members, families, guardians, Support Coordinators, providers, and DSPs to ensure quality services and DDD compliance. Maintain documentation, coordinate meetings and visits, and monitor certifications and deadlines. This role blends administrative precision with hands-on member support and local travel to member residences and facilities.

What You'll Do10

  • 1Coordinate services for members receiving Group Home and HCBS services
  • 2Maintain ongoing communication with members, guardians, families, Support Coordinators, providers, and agency staff
  • 3Monitor and maintain member and provider documentation to ensure compliance with DDD and agency requirements
  • 4Track employee and provider certifications, training requirements, expiration dates, and other compliance deadlines
  • 5Coordinate and follow up on member appointments, meetings, assessments, and required visits
  • 6Attend in-person planning meetings, member visits, staffing meetings, and other caseload-related appointments as needed
  • 7Travel to member residences, group homes, meeting locations, and other community locations when required
  • 8Complete required member visits and follow-up documentation within established timeframes
  • 9Review member files regularly to ensure documentation is complete, current, and properly maintained
  • 10Assist with HCBS and Group Home provider onboarding, documentation, and ongoing compliance

  • 1Strong organizational and time-management skills
  • 2Excellent written and verbal communication skills
  • 3Ability to manage multiple members, deadlines, appointments, and compliance requirements simultaneously
  • 4Must be dependable and demonstrate consistent follow-through
  • 5Ability to work independently while also collaborating with a team
  • 6Strong attention to detail and documentation
  • 7Proficiency with computers, email, electronic documentation, and scheduling systems
  • 8Prior experience in developmental disabilities, behavioral health, health care, case management, care coordination, or a related field is preferred
  • 9Knowledge of Arizona DDD requirements and HCBS services is preferred
  • 10Must have reliable transportation and be comfortable traveling locally to attend meetings, visits, and other caseload-related appointments as needed
