Director of Nursing, Home Care & Downstate Operations
Overview
RN licensed leader responsible for directing home care clinical operations across the Downstate region at scale. Own patient care quality, regulatory compliance, staff development, and operational excellence for a large clinical team of RNs, Clinical Coordinators, and caregivers. Partner with executive leadership on quality initiatives, audits, staffing strategy, and process improvement in a hands-on, fast-paced environment. This role blends clinical leadership with strategic program delivery to elevate home health services.
What You'll Do10
- 1Lead day to day nursing operations across the Downstate region to ensure high quality patient care.
- 2Recruit, onboard, train, and develop Registered Nurses and Clinical Coordinators to build a high performing clinical team.
- 3Maintain appropriate clinical staffing and workload distribution to meet patient needs.
- 4Conduct performance evaluations, coaching, and ongoing staff development to strengthen clinical capabilities.
- 5Oversee caregiver education, competency programs, and in service training across teams.
- 6Maintain compliance with NYS Department of Health regulations, accreditation standards, and agency policies.
- 7Lead internal audits, investigate incidents, and implement corrective action plans.
- 8Oversee complaint resolution and regulatory follow up to close gaps.
- 9Review clinical documentation to ensure quality and compliance with standards.
- 10Collaborate with Human Resources, Coordination, and Operations to improve clinical workflows and readiness for surveys and quality initiatives.
Requirements10
- 1Active New York State Registered Nurse (RN) license
- 2BSN required; MSN or Master’s in Healthcare Administration preferred
- 3Minimum 5 years of nursing experience including home care or community health
- 4At least 2 years of nursing leadership experience managing clinical teams
- 5Strong knowledge of New York home care regulations, DOH requirements, accreditation standards, and quality assurance
- 6Experience hiring, mentoring, and developing nurses and clinical staff
- 7Strong leadership, communication, and organizational skills
- 8Comfortable managing multiple priorities in a fast paced healthcare environment
- 9Proficient with EMR systems and Microsoft Office
- 10Collaborative leader with excellent clinical judgment and problem solving abilities
