- 1Develop and maintain strong relationships with key referral hospital personnel
- 2Identify new opportunities to expand market presence and advance growth
- 3Conduct daily rounds in assigned hospitals to identify new patients/residents needing post acute services
- 4Perform clinical assessments to evaluate care needs before admission
- 5Utilize a central intake process to generate new admissions into communities
- 6Communicate discharge plans to interdisciplinary teams based on accurate information from physicians and referrals
- 7Oversee referral processes and ensure patient/customer service excellence
- 8Direct staff education on admissions compliance and best practices
- 9Update documentation to reflect sales activity and maintain accurate records
- 10Prepare Business Development logs and plans weekly for distribution
- 11Facilitate strategic physician relationships and support physician advisory board development
Requirements10
- 1Minimum of one year of experience in a healthcare setting
- 2Strong clinical assessment skills
- 3Thorough understanding of Long-Term Care market and referral sources
- 4Proven history of meeting or exceeding census goals
- 5Working experience with an insurance company or private case management firm
- 6Excellent communication and organizational abilities
- 7Ability to read, write, and speak English fluently
- 8Travel capability via various transportation methods
- 9Essential functions include daily rounds and clinical evaluations
- 10Experience with Microsoft Office applications
