Overview

Direct Care Workers provide compassionate support to clients in residential facilities, assisted living, and home health settings. You will assist with daily living activities, monitor health status, and support independence and safety for individuals with disabilities or age-related needs. This role combines patient-centered care with teamwork to deliver high-quality, individualized support. You will work onsite in Phoenix, with competitive pay and benefits.

What You'll Do10

  • 1Build safe, clean, and supportive living environments through daily sanitation and safety protocols
  • 2Provide ADL assistance including bathing, dressing, grooming, and toileting to maintain client independence
  • 3Monitor vital signs and health changes to inform care plans
  • 4Assist with mobility using transfer methods and assistive devices such as Hoyer lifts
  • 5Document progress accurately and communicate with healthcare teams
  • 6Support transportation to medical appointments and community activities
  • 7Offer emotional support and companionship to enhance quality of life
  • 8Follow safety procedures including first aid, CPR, and emergency protocols
  • 9Assist with meal preparation per dietary restrictions and care plans
  • 10Maintain confidentiality and HIPAA compliance in all interactions

Requirements10

  • 1Previous experience in group homes, nursing homes, assisted living, or home health care preferred
  • 2Knowledge of dementia and memory care, neurodevelopmental disorders such as autism preferred
  • 3Ability to perform patient observation and manual handling tasks
  • 4Experience with medication administration and care planning
  • 5Strong communication skills for effective client interaction and documentation
  • 6Ability to perform heavy lifting and manual handling safely
  • 7Familiarity with HIPAA regulations
  • 8First aid and CPR certification desirable with ongoing training
  • 9Competence in patient transfer methods including Hoyer lifts
  • 10Geriatrics or long-term care knowledge
