You own the client journey from intake to ongoing care, managing relationships with 50+ families and coordinating caregiver schedules. You act as the primary liaison, bridging clients, caregivers, and internal teams. Your bilingual Spanish skills connect with diverse families, ensuring their needs are met with empathy and precision. What You'll Do10

  • 1Interview Spanish-speaking clients and families to capture care needs, preferences, and goals for personalized plans.
  • 2Build trust with 30+ active clients by providing weekly updates on schedules, care plans, and service changes.
  • 3Coordinate caregiver assignments using CareTime and ShiftWizard to ensure coverage for every client shift.
  • 4Conduct satisfaction surveys and analyze feedback to improve care quality, reducing escalations by 20%.
  • 5Serve as the agency's face at community health fairs, generating 15+ qualified referrals per quarter.
  • 6Maintain accurate client records in Kinnser and HHAeXchange to meet compliance standards.
  • 7Collaborate with nurses and administrative staff to resolve client issues within 24 hours of report.
  • 8Prepare weekly care reports for families, highlighting milestones and adjustments to care plans.
  • 9Facilitate client onboarding, completing paperwork and explaining services to 10 new clients monthly.
  • 10Partner with recruiting to interview caregiver candidates, assessing bilingual proficiency and fit.

Requirements8

  • 12+ years of scheduling experience in home care or healthcare setting.
  • 21-3 years in client relations, customer service, or healthcare administration.
  • 3Bilingual fluency in Spanish and English (written and spoken).
  • 4Knowledge of home health care services and industry regulations (e.g., HIPAA, OIG).
  • 5Proficiency in Microsoft Office (Outlook, Excel, Word) and EHR systems.
  • 6Strong interpersonal skills with ability to communicate empathetically under pressure.
  • 7Proven organizational skills managing multiple client cases and caregiver schedules.
  • 8Ability to work independently and collaboratively in a team-oriented environment.
