Certified Nursing Assistant (CNA) - Transitional Care
Overview
Provide hands-on patient care at PureHealth Transitional Care, reporting to licensed nurses in a CNA role. You will own daily living support for patients during 6A-2P, 2P-10P, or 10P-6AM shifts. Work with a dedicated nursing team focused on quality outcomes and patient dignity. What You'll Do10
- 1Deliver physical care: bathing, oral hygiene, grooming, incontinence care, toileting, colostomy care, transfers, ambulation, and range of motion exercises.
- 2Document patient care and observations in real-time using Par Scanning and ADL documentation systems.
- 3Communicate changes in patient condition or behavior to the licensed nurse on shift.
- 4Assist with meals: prepare patients, serve trays, feed residents, record oral intakes accurately.
- 5Transport patients to therapy appointments and support recreational activities.
- 6Respond to call lights within 90 seconds and complete regular rounding to anticipate needs.
- 7Uphold patient rights, including freedom from restraint and abuse; report any incidents immediately.
- 8Maintain patient room cleanliness and safety, adhere to infection control and isolation protocols.
- 9Keep uniform and personal appearance professional per facility standards.
- 10Complete annual continuing education and participate in interdisciplinary quality initiatives.
Requirements8
- 1High School Diploma or equivalent.
- 2Valid state CNA license from an approved nursing assistant certification course.
- 3Current BLS (CPR) certification.
- 41+ years of CNA experience preferred.
- 5Proficient in ADL documentation and real-time charting.
- 6Ability to perform physical tasks including lifting and transferring patients.
- 7Strong communication skills to interact with patients, families, and staff.
- 8Commitment to patient dignity and privacy.
