Case Planner, Juvenile Justice Services at Hope For Youth
Overview
Case Planner leads family-centered interventions for up to 20 youth in Nassau County, delivering case management, life coaching, and parent skills training. This field-based role requires weekly in-person contact with youth and families, including evenings and weekends. Collaborates with schools, courts, and community partners to support positive outcomes and reduce risk.
What You'll Do10
- 1Build and maintain weekly contact with youth and families via field visits
- 2Deliver case management, life coaching, and parent skill training to each caseload
- 3Coordinate with schools, service providers, and justice system representatives
- 4Conduct CANS 2.0 assessments and update service plans
- 5Attend school meetings and court appearances as needed to advocate for youth
- 6Complete timely documentation and case notes
- 7Participate in transition and discharge planning
- 8Attend supervision and ongoing trainings
- 9Review and adjust service plans to meet NYS and agency standards
- 10Ensure ethical conduct and adherence to program procedures and policies
Requirements7
- 1BSW or Bachelor's Degree plus 2 years of casework experience
- 2OR MSW or Master’s in a related field
- 3Excellent verbal and written communication skills
- 4Strong computer skills and ability to manage documentation
- 5Flexibility to work evenings and weekends
- 6Access to a reliable vehicle for home visits in Nassau County
- 7Bilingual (Spanish/English) preferred
