Overview

Case Manager role focused on shelter care and crisis intervention for at risk children and families. Owns client progress, intake, and service planning at scale within our shelter program. You will collaborate with internal teams and community agencies to advocate and coordinate emergency shelter, assessments, and follow-up support. What You'll Do10

  • 1Build and monitor client progress plans and provide coaching to families and youth to achieve goals
  • 2Design and manage service plans and goals with DCS case plans and CFTMs
  • 3Lead intake and needs assessments, classify crisis situations, and ensure RHYMIS/HMIS data accuracy
  • 4Collaborate with schools, shelters, and community partners to coordinate transportation, referrals, and crisis response
  • 5Contribute to placement updates and communicate progress with FCMs and network agencies
  • 6Support on call crisis response and staff crisis management as needed
  • 7Develop and maintain relationships with local social welfare agencies and shelter partners
  • 8Document case notes and prepare reports for DCS Probation and RHYMIS within required timeframes
  • 9Refer clients to community resources and monitor progress through ongoing evaluation
  • 10Assist with data tracking, confidentiality, and adherence to Children’s Bureau standards

Requirements9

  • 1Bachelor's degree in social work, counseling, psychology, or related field
  • 2Valid driver’s license and ability to travel in the community
  • 3Strong communication skills and responsiveness to cultural differences
  • 4Knowledge of social service systems within the state
  • 5Clear confidentiality practices and ability to maintain client privacy
  • 6Ability to develop and sustain professional networks with key contacts
  • 7Strong prioritization and time management abilities
  • 81-2 years of experience in child welfare or related field
  • 9Onsite ability in Indianapolis, Indiana and commitment to equal opportunity employment
