Bilingual Social Worker, Spanish Required - Hybrid SF & Community Services
Overview
The role owns coordination and advocacy for individuals with developmental disabilities age 3 and up, across SF County, balancing home visits, office time, and remote work. You will assess needs, develop and monitor Individual Program Plans, and manage a diverse caseload with a professional social work approach. You will collaborate with interdisciplinary teams, fund regional center services, and ensure timely documentation while advocating for entitled services and community resources. This role offers a hybrid schedule and a path to impact lives through comprehensive social support.
What You'll Do10
- 1Build and monitor Individual Program Plans and documentation to ensure timely outcomes for individuals and families.
- 2Design and adjust service plans by analyzing data and removing barriers to developmental objectives, coordinating with community resources.
- 3Own regional center funding processes, initiating and terminating funding as needed and ensuring purchased services are provided.
- 4Drive outreach and referrals to vendored services, evaluating availability and effectiveness of supports.
- 5Lead ongoing social assessments and case planning with interdisciplinary teams to support individuals.
- 6Collaborate with families to maximize participation in services and resources across community providers.
- 7Coordinate information collection for team use and prepare required reports with accuracy and timeliness.
- 8Develop and maintain compliant records in line with federal and state requirements.
- 9Advocate for individuals and families to access entitled services and navigate generic services as needed.
- 10Identify community service gaps and contribute to development of new services to improve quality of care.
Requirements8
- 1Bachelor’s Degree from an accredited school of social welfare or related field
- 2Master’s Degree preferred
- 3Fluency in Spanish with ability to speak and understand dialects as appropriate
- 4Knowledge of counseling techniques and family development
- 5Ability to work independently and as part of an interdisciplinary team
- 6Excellent business-like written communication and ability to convey complex information clearly
- 7Experience in case management is strongly desired
- 8Access to reliable transportation for program visits with short notice
