Overview
You will own wound management for acute and chronic cases at a skilled nursing facility in Detroit, collaborating with facility nurses and a multidisciplinary team. You'll assess patients, develop care plans, and perform advanced procedures like debridement and dressing changes, driving optimal healing outcomes for pressure ulcers, diabetic foot ulcers, and post-surgical wounds. What You'll Do9
- 1Conduct comprehensive patient assessments, including health histories and physical exams, to guide wound care decisions.
- 2Develop, implement, and adjust individualized treatment plans for complex wounds using evidence-based guidelines.
- 3Perform wound debridement, dressings, and advanced procedures to support healing and prevent infection.
- 4Order and interpret diagnostic tests, prescribing medications and treatments as appropriate.
- 5Educate patients and families on wound care techniques, prevention, and lifestyle changes to reduce recurrence.
- 6Collaborate with physicians, nurses, and therapists in multidisciplinary team meetings to ensure integrated care.
- 7Coordinate referrals to specialists and manage care transitions for continuity.
- 8Participate in quality improvement initiatives and contribute to updating wound care protocols.
- 9Maintain accurate patient records and ensure compliance with regulatory standards.
Requirements7
- 1NP, PA, or PT license in Michigan with wound care certification preferred (e.g., CWCN, CWS, WCC).
- 22+ years clinical experience in wound management across acute and chronic settings.
- 3Proficiency in debridement, dressing selection, and advanced wound care modalities.
- 4Strong knowledge ofpressure ulcers, diabetic foot ulcers, venous ulcers, and post-surgical wounds.
- 5Ability to work onsite in Detroit with flexible hours.
- 6Excellent communication and collaboration skills for multidisciplinary care.
- 7Commitment to continuing education and evidence-based practice.
