- 1Support clinical operations for Assisted Living and Home Care programs
- 2Ensure resident care plans are accurate individualized and updated
- 3Monitor resident care to guarantee services follow physician orders
- 4Provide day-to-day clinical guidance coaching and mentoring to caregivers
- 5Support medication administration practices and documentation standards
- 6Conduct chart audits and quality reviews to uphold nursing practices
- 7Participate in resident assessments move-ins and interdisciplinary care planning
- 8Communicate with residents families physicians pharmacies hospice providers and healthcare professionals
- 9Assist with regulatory compliance surveys and quality improvement initiatives
- 10Maintain clinical supplies and equipment availability
- 11Collaborate with Director of Health Services Clinical Nurse Manager and Home Care Manager on staffing and operations
- 12Join nurse on-call rotation and respond to clinical emergencies
Requirements7
- 1Current Colorado Registered Nurse license
- 2Experience in Assisted Living Home Care Senior Living or Skilled Nursing preferred
- 3Strong nursing assessment and critical thinking skills
- 4Excellent communication and relationship-building abilities
- 5Passion for mentoring caregivers and promoting high-quality resident care
- 6Strong organizational skills and attention to detail
- 7Ability to work independently while collaborating with interdisciplinary teams
