Transplant Nurse Coordinator, Johns Hopkins Hospital
Overview
You will own transplant patient coordination at Johns Hopkins Hospital, managing testing, education, and multidisciplinary care for a high-acuity transplant population. You will operate within a BLS-certified nursing framework, working alongside a dedicated transplant team to optimize patient outcomes. This role combines direct patient assessment with leadership and data management for regulatory reporting. You will also participate in a triage call rotation, ensuring emergent patient needs are addressed promptly.
What You'll Do8
- 1Coordinate transplant patient care from initial evaluation through post-transplant follow-up, ensuring all required testing and clinical indicators are tracked and completed.
- 2Assess patient health status using diagnostic procedures and lab reviews, identifying issues and escalating as needed to the multidisciplinary team.
- 3Educate patients and families on transplant protocols, medications, and lifestyle changes, using EPIC to document and track education.
- 4Partner with surgeons, nephrologists, and social workers to develop and execute individualized care plans for each transplant candidate and recipient.
- 5Collect and maintain patient data for clinical research and regulatory reporting, ensuring compliance with UNOS standards.
- 6Mentor junior nurses and support staff, promoting evidence-based practice and quality improvement initiatives across the transplant unit.
- 7Triage emergent patient calls during on-call rotations, prioritizing urgent needs and coordinating appropriate interventions.
- 8Lead weekly transplant team meetings to review patient progress, adjust care plans, and address barriers to successful outcomes.
Requirements8
- 1Bachelor of Science in Nursing (BSN) required
- 2Active Maryland or Compact RN license required
- 3BLS certification required
- 43-5 years of acute care RN experience required
- 5Transplant nursing experience preferred
- 6Case management and/or patient education experience preferred
- 7Strong clinical assessment and critical thinking skills, with the ability to work independently in a fast-paced environment
- 8Excellent communication and collaboration abilities for multidisciplinary teamwork
