What You'll Do11

  • 1Coordinate patient care for heart transplant across all campuses including referral evaluation and waitlist management
  • 2Collaborate with stakeholders to facilitate hospital transfers and coordinate multidisciplinary care
  • 3Provide clinical oversight and escalate patient issues to transplant stakeholders
  • 4Anticipate and facilitate diagnostic testing imaging and treatment plans
  • 5Serve as liaison between patients families and multidisciplinary transplant team
  • 6Participate in multidisciplinary rounds and case conferences
  • 7Navigate Phoenix and Epic systems to access patient charts
  • 8Deliver individualized patient and family education on transplant processes
  • 9Ensure regulatory compliance with UNOS CMS and Joint Commission standards
  • 10Complete required transplant forms and documentation promptly
  • 11Travel to Hillcrest and East Campuses for program expansion

Requirements11

  • 1Graduate from accredited nursing school with current RN license from California
  • 2BART certification within six months of hire date
  • 3Minimum one year pre-transplant coordinator experience with direct patient care
  • 4Proven experience coordinating multidisciplinary care across inpatient outpatient settings
  • 5Understanding of transplant phases UNOS/OPTN guidelines and CMS regulations
  • 6Strong critical thinking and problem solving skills for complex cases
  • 7Excellent interpersonal skills for influencing multidisciplinary teams
  • 8Ability to adapt to fast-paced environment and workflow changes
  • 9Commitment to UCSD Health CARINIG standards of performance
  • 10BSN preferred with transplant or organ procurement experience
  • 11Epic and Phoenix EHR proficiency required
