Overview
As the Transitional Housing Case Manager at A Safe Place, you will own the full lifecycle of securing and placing survivors of domestic violence, dating violence, human trafficking, sexual assault, and stalking into transitional housing and rapid rehousing programs across Lake County, Illinois. You will build landlord partnerships, conduct VISPDAT assessments, and coordinate client services including employment training, counseling, and legal advocacy. Reporting to the Associate Director of Crisis Housing, you will work with a dedicated team in a 24/7 on-call rotation. This role offers a direct impact on survivor safety and empowerment in a mission-driven nonprofit environment.
What You'll Do10
- 1Build and maintain landlord partnerships, educating landlords on safety protocols and relevant laws like the Illinois Domestic Violence Act and Orders of Protection.
- 2Manage housing applications, including income and household size updates, and re-certifications to ensure client housing stability.
- 3Oversee temporary housing for two families in units leased by A Safe Place.
- 4Perform outreach to community organizations, housing resources, and landlord associations to expand housing opportunities.
- 5Coordinate intake and eligibility assessments, including housing assessments and VISPDAT evaluations.
- 6Create individualized service plans and deliver employment skills training, education, economic assistance, counseling, children's services, and legal advocacy.
- 7Implement assessment tools and comply with funder reporting requirements, ensuring accurate and timely documentation.
- 8Participate in the on-call rotation (7 days) and work one weekend every 4-6 weeks to support the crisis response team and HT House.
- 9Attend monthly Lake County Homeless Coalition meetings and collaborate with internal and external referral partners.
- 10Recruit and develop community and corporate partnerships, providing training as needed.
Requirements10
- 1Bachelor's degree in Social Work, Counseling, Education, or closely related field.
- 2Flexible schedule to meet client needs and respond to crisis, including 24/7 on-call rotation.
- 3Bilingual preferred.
- 4Proven ability to motivate clients and support their progress toward independence.
- 5Excellent written and oral communication skills, including documentation and group facilitation.
- 6Strong interpersonal skills demonstrating genuine concern and respect for clients and staff.
- 7Ability to work independently and as part of a team, exercising mature judgment.
- 8Highly motivated self-starter capable of coordinating multiple projects and tasks.
