Overview

You will own case management for residents in a senior apartment building, coordinating home and community-based services across Chicago. You partner with the Housing Manager and community contacts to arrange educational programs and ensure HUD compliance. This part-time role offers $22/hour and flexible weekday hours, with training and growth in a mission-driven nonprofit. You make a direct impact on older adults' stability and well-being.

What You'll Do7

  • 1Conduct needs assessments for new and existing residents, identifying service gaps and crisis situations within the first 90 days.
  • 2Deliver crisis intervention and ongoing case management, maintaining strict confidentiality per HIPAA and LSSI policy.
  • 3Build a community resource network, acting as the liaison between residents and local agencies for entitlements and support.
  • 4Design and schedule educational programs for residents and staff, covering topics like fall prevention and Medicare benefits.
  • 5Maintain resident files and documentation to meet HUD and LSSI standards, submitting all reports on time.
  • 6Collaborate with the Housing Manager to address building-level resident needs and service coordination.
  • 7Model professional conduct, treating residents and colleagues with dignity and respect in every interaction.

Requirements7

  • 1Bachelor’s degree in human services or related field required.
  • 2Background check clearance required before start.
  • 3Experience working with older adults and knowledge of entitlements preferred.
  • 4Valid driver’s license in good standing for Illinois, with reliable transportation and IL minimum liability insurance.
  • 5Microsoft Office proficiency: Outlook, Teams, Word, Excel, and PowerPoint.
  • 6Strong verbal and written communication, plus organizational and presentation skills.
  • 7Flexibility to work Monday-Friday, with part-time hours.
