Supportive Housing Program Coordinator
Overview
The Supportive Housing Program Coordinator leads daily supervision of staff volunteers and interns. This role supports report submission data monitoring training and grant compliance. It involves close collaboration with Program Managers for program planning and evaluation. The position provides direct case management for adults experiencing chronic homelessness aiming for housing stability.
What You'll Do12
- 1Lead case management services for diverse populations including mental health and substance abuse disorders
- 2Conduct client screenings and determine eligibility for housing services
- 3Facilitate access to mental health substance abuse and physical health services
- 4Develop and monitor individual housing services plans focused on stability
- 5Transport clients to agencies and provide crisis intervention when needed
- 6Work with Program Managers on program development monitoring and performance management
- 7Assist with budget related tasks and ensure compliance with fiscal policies
- 8Collaborate with ECHO for Coordinated Entry referral process and intake oversight
- 9Partner with Employment Specialists and Education Specialists to link clients to opportunities
- 10Maintain accurate records in Service Point HMIS database
- 11Prepare reports for internal and funding sources
- 12Engage in community housing activities and participate in training sessions
Requirements9
- 1Minimum 2 years working directly with low income diverse underserved populations
- 2Minimum 1 year supervisory experience with preference for housing programs or homelessness work
- 3Bachelor's degree in social work or related field
- 4Master's degree or related licensure preferred
- 5At least 5 years of experience with CMS certification or similar
- 6Valid Texas driver's license within 30 days of employment
- 7Proficiency with Excel databases and HMIS software
- 8Bilingual English Spanish skills preferred
- 9Attendance at Caritas of Austin staff meetings and training sessions required
