Social Worker/QP Minneapolis
Overview
Lead housing placement and case management for Twin Cities clients. Develop individualized transition plans coordinate community resources supervise staff and ensure needs are met. Maintain accurate records provide discharge support and liaise with support teams.
What You'll Do10
- 1Assist clients in locating appropriate housing options
- 2Coordinate with service providers and families for transition plans
- 3Perform client intake and regular assessments
- 4Support discharge planning and housing coordination
- 5Maintain client records using Microsoft 365 and Teams
- 6Provide discharge assistance including light lifting
- 7Consult on medical assistance and waiver programs
- 8Manage community resource programs such as CADI and Elderly Waiver
- 9Complete case documentation and assist during discharge days
- 10Drive vehicles for client transport with valid license
Requirements10
- 1Licensed Social Worker in Minnesota
- 2Current CPR certification preferred
- 3Valid driver’s license with good record
- 4Bachelor’s degree in Social Work or related field
- 5Two years experience in human services or healthcare
- 6Knowledge of community resources and county applications
- 7Ability to perform intake and regular assessments
- 8Experience with discharge planning and housing coordination
- 9Background check clearance for human services work
- 10Reliable transportation and professional demeanor
