Social Worker, Aging & Housing Support in Bronx
Overview
Lead clinically to support seniors in a supportive housing setting with a focus on housing stability and quality of life. You will deliver trauma-informed, strengths-based services including psychosocial assessments, crisis intervention, and care coordination at scale across a diverse resident population. You will collaborate with interdisciplinary teams, medical and community partners to reduce readmissions and promote independence. What You'll Do10
- 1Build comprehensive psychosocial assessments and collaborate on individualized support plans
- 2Provide crisis intervention and stabilization as needed to prevent deterioration
- 3Coordinate with medical providers, mental health professionals, home care agencies, and community organizations
- 4Advocate for residents within healthcare, social service, and legal systems
- 5Support hospital discharge planning and case conferences to improve outcomes
- 6Maintain accurate, timely documentation in compliance with agency and funder requirements
- 7Deliver monthly staff trainings on the impact of mental health, substance use, and medical barriers
- 8Collaborate with Recreational Specialist to support psychosocial programming
- 9Identify program needs and make recommendations for service improvements
- 10May require after-hours or weekend follow-up to emergency situations
Requirements10
- 1Current active Social Work license in good standing (e.g., LMSW, LCSW)
- 2Master's degree in social work from CSWE-accredited program or CASAC
- 3NY licensure with at least two years in a specialized social work practice
- 4Minimum 2–3 years of experience with formerly homeless individuals or residents in supportive housing
- 5Strong knowledge of aging-related physical, cognitive, and psychosocial conditions
- 6Experience with trauma-informed, harm reduction, and strength-based models
- 7Ability to conduct risk assessments and crisis intervention
- 8Excellent documentation and organizational skills
- 9Strong interdisciplinary collaboration skills
- 10Fluency in Spanish is highly desirable
