Service Coordinator, Early Intervention & Family Services
Overview
You will own service coordination for families eligible for New Jersey Early Intervention Services, ensuring timely delivery of authorized services across South Jersey. You will partner with a multidisciplinary team of healthcare professionals and community providers to maximize child development outcomes. This role combines case management, advocacy, and cross-agency collaboration. You will make a direct impact on maternal and child health in a nonprofit dedicated to equity and quality improvement.
What You'll Do8
- 1Facilitate development, review, and evaluation of individualized family service plans with family input
- 2Coordinate family assessments and evaluations using approved program criteria and data collection tools
- 3Act as point of contact for families to identify service providers and community resources
- 4Monitor service delivery and advocate for families to ensure timely access to authorized services
- 5Develop transition plans to school-based services for children aging out of early intervention
- 6Document all service coordination activities accurately and completely
- 7Maintain confidentiality and provide culturally competent services to diverse populations
- 8Collaborate with agency staff and leaders to share information and improve program outcomes
Requirements7
- 1Bachelor's degree required
- 2Meet NJEIS Personnel Standards for Service Coordinator
- 31+ year experience working with children birth to three
- 4Basic computer skills in Word and Excel
- 5Skills in case management, community outreach, and parenting education
- 6Valid driver's license, current insurance, and reliable car
- 7Bilingual English/Spanish preferred
