Service Coordinator - Autism Support
Overview
The Service Coordinator at San Gabriel/Pomona Regional Center leads case management for children ages 0 to 3 and families. This role drives compassionate support through collaboration with community partners. You will develop Individualized Family Service Plans and coordinate services across multiple domains. What You'll Do9
- 1Assist children ages 0 to 3 with developmental disabilities and their families in locating appropriate community services and programs
- 2Consult with Manager and specialist staff to coordinate services including medical behavioral health psychological educational vocational recreational adaptive devices
- 3Explain available resources and services to individuals served and their families ensuring understanding of options
- 4Collaborate to assess and address children's needs through in-person meetings with families and agencies
- 5Develop and implement individualized Family Service Plans tailored to each child's unique needs
- 6Document service plans meetings and progress accurately while meeting mandated reporting timelines
- 7Initiate purchase of service through the Regional Center for eligible clients
- 8Present cases at specialized committees when required
- 9Perform additional duties supporting departmental and organizational goals
Requirements12
- 1Bachelor's degree in social work psychology human development sociology public health nursing or related field
- 2At least one year experience working with individuals with developmental disabilities or related field
- 3Master's degree in related field may substitute for required experience
- 4Bilingual Spanish proficiency preferred
- 5Knowledge of developmental disabilities and community resources
- 6Strong interpersonal communication skills both verbal and written
- 7Ability to develop and implement individualized support plans adapt strategies independently
- 8Excellent computer skills in MS Office including Word and Outlook
- 9Proven ability to multitask prioritize assignments and meet deadlines with conflicting priorities
- 10Compassionate culturally sensitive approach to diverse populations
- 11Valid driver's license reliable transportation and minimum liability insurance
- 12Physical ability to walk stand drive and lift up to 25 pounds
