- 7Mobilizes community agencies and support networks for patients
- 8Documents social work activities according to departmental protocols
- 9Performs clinical duties independently with appropriate supervision
- 10Assists in ensuring service delivery aligns with policies
- 11Regularly participates in supervision and prepares statistical documentation
- 12Presents psychosocial information in interdisciplinary rounds and meetings
- 13Provides supervision to social work interns
- 14Conducts patient/family orientation sessions
- 15Participates in interdisciplinary planning and unit meetings
- 16Develops programs and coordinates case reviews in specialized areas
- 17Serves as departmental resource and committee representative
- 18Organizes training for new staff and transfers
- 19Leads psychodynamic support groups for patients and families
- 20Represents Social Work Department in hospital and community committees
- 21Conducts chart reviews to verify policy compliance
Requirements7
- 1Masters Degree in Social Work or equivalent from CSWE accredited program
- 2Licensed Clinical Social Worker license in New York State
- 3Minimum four years post‑LMSW experience in healthcare settings
- 4If working in Gender Affirming Surgery two years experience in outpatient mental health with LGBTQIA+ population required
- 5If working in Palliative Care post‑master’s certificate or fellowship in palliative care needed
- 6Ability to communicate effectively across organizational levels
- 7Previous experience in hospital or health‑related organization preferred
