What You'll Do10
- 1Build patient care plans and coordinate with physicians to meet active plans of care
- 2Deliver on-site pediatric nursing care in patients' homes with 1:1 patient care ratio
- 3Administer medications treatments and therapies as prescribed
- 4Educate family members on patient clinical care to improve outcomes
- 5Coordinate care across the patient’s home health team and resources
- 6Perform ongoing assessments and monitoring to ensure quality of care
- 7Support preventative initiatives to uphold patient safety and quality care
- 8Engage in ongoing clinical education and professional growth opportunities
- 9Participate in 24/7 clinical support and SIM lab trainings
- 10Contribute to a positive, patient-centered care culture within the team
Requirements5
- 1Active RN or LPN license in Arizona
- 2Ability to provide home health care to infants children and adolescents
- 3Willingness to work full-time or part-time day night or weekend shifts
- 4Training provided for new grads
- 5Commitment to compassionate patient care and positive family interactions
