Overview
You own restorative care for residents at Landmark of Hyde Park in Chicago, ensuring each person maintains or gains physical, mental, and emotional function. Working under the Restorative Nurse and DON, you execute individualized plans to prevent avoidable decline and promote independence. Your team includes nurses, therapists, and CNAs in a supportive onsite environment. This role offers direct patient impact and daily variety in a skilled nursing setting.
What You'll Do8
- 1Deliver restorative nursing interventions per each resident's plan of care, targeting mobility, ADLs, and range of motion.
- 2Document care provided and resident responses in the EMR within 24 hours.
- 3Lead restorative exercise sessions such as ambulation, transfer training, and passive/active ROM.
- 4Collaborate with the Restorative Nurse to update care plans based on progress or setbacks.
- 5Coach residents on safe techniques for bed mobility, toileting, and self-feeding to boost independence.
- 6Report changes in condition to the DON or charge nurse immediately.
- 7Maintain restorative equipment and supply inventory, including gait belts and therapy bands.
- 8Assist with admission and discharge restorative assessments under supervision.
Requirements8
- 11+ years of restorative aide or CNA experience in skilled nursing or rehab.
- 2Active CNA certification in Illinois or completion of a restorative aide training program.
- 3Hands-on skill with gait training, transfer techniques, and range-of-motion exercises.
- 4Ability to lift 50+ pounds and assist residents with mobility tasks.
- 5Documentation proficiency in electronic medical records (e.g., PointClickCare).
- 6Current CPR certification within 90 days of hire.
- 7Must pass a background check and physical exam.
- 8Availability for 8am to 4pm shifts, including weekends on rotation.
