Rehabilitation Case Manager Brain Injury, Healthcare & Case Management
Overview
CRC or CCM background welcomed. Own client advocacy and care coordination for adults with brain injuries at scale across multiple sites. Lead interdisciplinary teams to develop and adjust individualized rehabilitation plans from admission to discharge. Bridge clinical teams, families, physicians, and funders to ensure cohesive, high quality care and progress toward goals. What You'll Do10
- 1Build and lead interdisciplinary treatment team meetings to coordinate client care across all phases
- 2Design and monitor individualized rehabilitation goals with the Clinical Director and primary providers to drive progress
- 3Maintain transparent stakeholder communication with clients, families, and funders regarding progress and care expectations
- 4Drive funding coordination by monitoring guidelines, exploring alternative sources, and ensuring accurate documentation for billing integrity
- 5Document clinical activity accurately in client charts, supervise staff notes, and ensure adherence to state standards and guidelines
- 6Support direct client care and transportation as needed and participate in the on call rotation
- 7Advocate for client needs throughout the care journey to optimize outcomes
- 8Monitor caseloads and adjust plans to align with evolving patient needs and funding changes
- 9Coordinate discharge planning and transition to appropriate community or post-acute supports
- 10Ensure compliance with regulatory requirements and professional standards throughout the case management process
Requirements7
- 1Bachelor’s degree in a healthcare or human services field
- 2Master’s degree preferred
- 3CRC or CCM certification preferred
- 4Proven case management experience in healthcare with crisis management, treatment planning, and needs assessment
- 5Valid driver’s license and clean driving record
- 6Strong writing, organizational, time management, and interpersonal communication skills
- 7Ability to lift/move 50+ lbs and travel up to 30% of the time
