Psychiatric Rehabilitation Program Case Manager General Description
Overview
The PRP Case Manager oversees case management services for individuals recovering from mental illness. They collaborate with service teams and providers to ensure comprehensive support. Full time position reporting to the Program Director.
What You'll Do9
- 1Coordinate program services including Residential Psychiatric Rehabilitation Program Health Home and Supported Employment
- 2Assist individuals in developing and tracking long term and short term goals
- 3Schedule and accompany individuals to medical and behavioral health appointments
- 4Complete contact notes in EHR and Individual Service Plans
- 5Communicate with Intake Coordinator Residential Supervisor and Program Director
- 6Provide off site services based on individual preference
- 7Utilize computer systems for external and internal communication
- 8Participate in Utilization Review Committee and related meetings
- 9Manage crisis situations and join emergency on call rotation
Requirements7
- 15+ years building ETL pipelines with Spark and Airflow
- 23-5 seasons leading mental health case management teams
- 3AA degree with at least two (2) years experience in mental health/psychiatric rehabilitation
- 4Bachelor`s Degree plus one (1) year experience in mental health/psychiatric rehabilitation
- 5Effective verbal and written communication skills
- 6Good driving record
- 7Ability to work as part of a team and independently

$44 - $48k per year
