We seek a skilled Family Care Coordinator to lead psychiatric rehabilitation for women post-incarceration. The role involves delivering community-based services to promote wellness and recovery. Candidates must drive therapeutic outcomes through evidence based practices.
What You'll Do5
- 1Facilitate psychiatric rehabilitation and behavioral support services per Individualized Rehabilitation Plan
- 2Teach life skills and encourage positive change through skill demonstrations
- 3Promote wellness and self management to achieve self sufficiency goals
- 4Expose clients to academic vocational and employment training resources
- 5Collaborate with therapists and nurses to ensure treatment adherence
Requirements5
- 1Minimum 3+ years direct support for adults with severe mental illness and substance abuse
- 2Preferable 1+ year experience with women released from incarceration
- 3High School Diploma or higher degree
- 4Valid driver’s license and reliable transportation
- 5Current COVID vaccination or first dose scheduled
