Medical Case Manager I, Nursing & Case Management in Healthcare
Overview
Own and manage complex workers compensation cases at scale across a local region. You will coordinate patient care, collaborate with medical providers, and drive outcomes for injured workers within a defined jurisdiction. This role blends in-person and telephonic case management with regular travel in the Atlanta area and surrounding counties. You will work with a cross functional team to develop effective long term care strategies and ensure appropriate, cost effective care.
What You'll Do10
- 1Build and manage a caseload of workers compensation cases with regular local travel
- 2Provide in person and telephonic case management to patients, physicians, providers, employers, and referral sources
- 3Evaluate treatment plans for medical necessity and cost effectiveness and discuss alternatives with physicians
- 4Attend discharge planning conferences and attend provider and attorney visits as needed
- 5Negotiate delivery of durable medical equipment and nursing services for home care
- 6Conduct home visits for initial and ongoing evaluation and care planning
- 7Collaborate with a multidisciplinary team to develop long term care strategies and monitor patient progress
- 8Maintain accurate documentation and case records using Microsoft Office tools
- 9Communicate clearly with patients, families, providers, and clients to coordinate care
- 10Meet travel requirements up to 60 with possible overnight travel and on call as needed
Requirements10
- 1Registered nurse license for the state of operation
- 2National certification such as CCM, COHN, CRRN, or CDMS preferred
- 3Experience as an RN in medical case management or related clinical background in orthopedics, neurology, or rehabilitation preferred
- 4Ability to travel to assigned locations and drive or use transportation with a valid driver’s license
- 5Strong communication, time management, and organizational skills
- 6Proficiency with Microsoft Office including Excel
- 7Ability to work independently and in a team
- 8Experience with utilization review or managed care is helpful
- 9Must be able to work in a high volume, fast paced environment
- 10Understanding of cost containment practices in healthcare
