As Rehab Manager, own rehabilitation services across a multi-disciplinary home health team at AMBER HOME HEALTH CARE in Detroit, MI. - 5Monitor therapy outcomes and implement quality improvement initiatives to elevate care standards.
- 6Collaborate with nursing, physicians, and other disciplines to deliver holistic patient care and resolve complex cases.
- 7Address patient or family concerns related to rehab services with prompt, solution-focused communication.
- 8Track and analyze productivity and visit rates to meet budget targets, reporting key metrics to senior leadership.
- 9Lead team meetings and educational sessions to promote evidence-based practices and regulatory updates.
Requirements6
- 1Active license as a Physical Therapist, Occupational Therapist, or Speech Therapist (required).
- 23-5 years of clinical experience in home health or rehabilitation settings.
- 3Prior supervisory or management experience in a healthcare environment.
- 4Working knowledge of Medicare, OASIS, and home health regulations.
- 5Strong communication and leadership skills to guide a multidisciplinary team.
- 6Experience with Kinnser EMR is a plus.
