Overview
You will own patient care plans for a diverse inpatient population at Jefferson Health's Cherry Hill campus, delivering patient- and family-centered care across the night shift. What You'll Do10
- 1Develop individualized nursing plans of care with patients and their care partners, prioritizing cultural beliefs and values.
- 2Integrate Evidence-Based Practice and clinical expertise with patient preferences to deliver optimal healthcare.
- 3Partner with patients to identify learning needs and provide education using patient education techniques.
- 4Minimize risk of harm by following established nursing protocols and documenting health information in electronic health records.
- 5Use data to monitor care outcomes and test changes to improve quality and safety.
- 6Function effectively in inter-professional teams, fostering open communication and shared decision-making.
- 7Act as a preceptor to appropriate staff and participate in hospital committees focused on safety and quality improvement.
- 8Perform patient assessment and diagnosis using clinical judgement, exploring symptoms and generating resolutions.
- 9Administer medications and manage prescriptions for continuing treatment, following medication administration protocols.
- 10Plan discharges by partnering with patients and families to arrange follow-up care.
Requirements9
- 1Diploma from an accredited school of nursing required; BSN or advanced degree within 3 years strongly preferred.
- 2Active RN license in the state of practice; BLS certification from American Heart Association or Red Cross.
- 31 year previous experience as an RN in an acute care setting strongly preferred.
- 4Knowledge of patient assessment and diagnosis, including tools and techniques for clinical judgement.
- 5Knowledge of medication administration and prescription management for inpatient and post-discharge care.
- 6Knowledge of health information documentation methods, including electronic medical records.
- 7Knowledge of discharge planning and patient education techniques for diverse populations.
- 8Physical ability to lift and carry 40-50 lbs, push/pull patients on beds and stretchers, and stand/walk frequently.
- 9Effective communication skills and cultural awareness to convey respect and connect with diverse patients and families.
