Overview
Full time 36 hours per week day shift professional nurse leads patient centered care using the nursing process addressing unique individual needs and cultural beliefs. Works collaboratively with interdisciplinary teams to deliver compassionate inclusive care while driving continuous improvement through data informed decisions.
What You'll Do11
- 1Interact consistently with co workers patients families and designated care partners aligning actions with Jefferson values
- 2Integrate evidence based practice clinical expertise and patient preferences to design optimal care pathways
- 3Partner with families to identify learning needs and provide understandable education fostering understanding and affirmation
- 4Minimize harm by adhering to established nursing protocols and performing essential physical tasks such as lifting patients and CPR
- 5Utilize information technology to communicate manage knowledge mitigate errors and support ethical decisions
- 6Analyze care outcomes using data to implement improvement methods enhancing quality and safety
- 7Lead primary preceptor role mentoring staff and actively participating in safety and quality improvement committees
- 8Give and receive feedback integrate changes and serve as preceptor for appropriate team members
- 9Actively engage in hospital settings based committees and demonstrate willingness to adapt practices based on feedback
- 10Develop discharge plans in partnership with patients and families ensuring continuity of care
- 11Conduct thorough assessments diagnose conditions and document all aspects of patient care in electronic records
Requirements16
- 1Specialized diploma graduate of accredited nursing school
- 2Bachelor of Science in Nursing degree required
- 3Patient and family centered practice knowledge
- 4Cultural awareness and ability to connect with diverse populations
- 5Effective communication skills to transmit receive and interpret information appropriately
- 6Knowledge of patient assessment and care principles
- 7Education background for child and adult patients including learning priority elicitation
- 8Medication administration and prescription knowledge
- 9Discharge planning expertise for post hospital needs
- 10Assessment and diagnosis abilities using available tools and techniques
- 11Proficiency in health information documentation and recording
- 12Basic Life Support certification and American Heart Association training
- 13Licensed Registered Nurse in Pennsylvania state
- 14CCRN or ECCO certification required or enrollment in adult critical care course
- 15Bachelor degree preferably within three years of hire
- 16One year prior RN acute care experience strongly preferred
