RN Med/Surg Telemetry, 10 Pavilion Center City
Overview
You will own patient care on the 10 Pavilion Med/Surg/Tele unit at Thomas Jefferson University Hospital in Center City Philadelphia, delivering patient- and family-centered care to a diverse patient population. You will partner with patients and their designated care partners to develop individualized care plans, coordinate care across inter-professional teams, and drive quality and safety outcomes. This rotating shift role offers the chance to precept new staff and lead unit committees while growing your clinical expertise in a nationally ranked academic medical center. You will use evidence-based practice and health information technology to optimize outcomes and support continuous improvement.
What You'll Do10
- 1Partner with patients and their families to develop individualized care plans that honor cultural beliefs and values, using patient- and family-centered care principles.
- 2Deliver safe, compassionate care by following established nursing protocols and using clinical judgment to assess, diagnose, and treat patients on Med/Surg/Tele.
- 3Educate patients and designated care partners on conditions, treatments, medications, and recovery steps using techniques that meet each learner's needs.
- 4Use health information technology to document care, communicate with the team, manage knowledge, and support ethical decision-making.
- 5Monitor patient outcomes with data and implement quality improvement methods to enhance safety and care quality.
- 6Coordinate discharge planning with patients and families, arranging follow-up care and ensuring smooth transitions.
- 7Collaborate with nursing and inter-professional teams, fostering open communication and shared decision-making.
- 8Precept new staff and serve as a resource for team members, promoting professional growth and development.
- 9Participate in hospital and unit-based committees focused on safety and quality improvement.
- 10Give and receive feedback to continuously improve practice and integrate changes into care delivery.
Requirements10
- 1Graduate of an accredited school of nursing with a Specialized Diploma in nursing, required.
- 2Active RN - Licensed Registered Nurse_PA - State of Pennsylvania license, required.
- 3BLS - Basic Life Support from AHA - American Heart Association, required.
- 41 year of experience as an RN in an acute care setting, strongly preferred.
- 5Bachelor’s Degree in BSN or completion of BSN or appropriate advanced degree within 3 years of hire, preferred.
- 6Knowledge of patient- and family-centered practice and cultural awareness.
- 7Effective communication and delegation skills, using the language of partnership.
- 8Proficiency in patient assessment, medication administration, and discharge planning.
- 9Ability to document health information in written or electronic form.
- 10Physical ability to lift and carry 40-50 lbs, push/pull patients on beds or stretchers, and stand or walk for extended shifts.
