  • 6Ensure medical behavioral and clinical components of ISP completion
  • 7Lead Person Centered Planning meetings and coordinate interdisciplinary team efforts
  • 8Oversee new staff orientation and act as liaison with families and agencies
  • 9Complete Mental Health Assessments and attend psychiatric appointments
  • 10Supervise waiver authorizations and manage individual fund administration
  • 11Coordinate service delivery and maintain detailed case records

Requirements11

  • 15+ years experience working with Developmental Disabilities
  • 23-5 years progressive management experience
  • 3Bachelor’s degree in Human Services or related field
  • 4Valid driver’s license and completed background screening
  • 5Cultural sensitivity demonstrated through partnership with clients and families
  • 6Ability to collaborate with interdisciplinary teams and external agencies
  • 7Strong written communication skills for report writing and training documentation
  • 8Proficiency in reading technical manuals and procedural guides
  • 9Experience with First Aid CPR and incident reporting systems
  • 10Capacity to lift up to 25 pounds and stand/walk/sit regularly
  • 11Frequent interaction with residents and staff requiring interpersonal communication
