Primary Care NP, Outpatient Tacoma & Preventive Medicine
Overview
The NP will own outpatient primary care for adults in a value-based, team-based setting in Tacoma. You’ll work with an interdisciplinary team including an onsite Health Coach to deliver comprehensive, preventive, long-term care at a moderate patient load. This role emphasizes relationship-based care, population health, and quality outcomes in a clinic with accessible lab and pharmacy services.
What You'll Do10
- 1Build comprehensive primary care plans for adult patients and manage acute and chronic conditions within NP scope
- 2Design and implement individualized treatment plans aligned with patient goals
- 3Prescribe and monitor medications and interpret diagnostic tests
- 4Educate patients on disease prevention, lifestyle choices, and self-management
- 5Collaborate with physicians, Health Coaches, and care team to coordinate care
- 6Utilize onsite lab and pharmacy services to improve efficiency and outcomes
- 7Participate in population health initiatives and quality improvement activities
- 8Maintain accurate EMR documentation and participate in committee work
- 9Support organizational goals related to preventive care and clinical outcomes
- 10Develop long-term patient relationships through patient-centered care
Requirements10
- 1Master's or Doctoral degree in Nursing (MSN, DNP, or equivalent)
- 2Washington State NP license or ability to obtain
- 3Board certification as FNP, AGPCNP, or other primary care NP
- 4National NP certification from an accredited organization
- 5Minimum of 3 years of primary care experience preferred
- 6Current DEA registration or ability to obtain
- 7Current BLS certification
- 8Negative TB test within 12 months of start
- 9Experience in team-based care and EMR use (Athena preferred)
- 10Proficiency with Google Workspace
