What You'll Do9

  • 1Build comprehensive PT evaluations and develop individualized plans of care
  • 2Improve mobility, strength, balance, safety, and functional independence through skilled interventions
  • 3Monitor resident progress and adapt treatment plans as needed within 90 days
  • 4Educate residents, families, caregivers, and staff on therapy goals and techniques
  • 5Collaborate with physicians, nursing, OT, speech therapy, and other team members to coordinate care
  • 6Maintain accurate timely documentation complying with facility, state, federal, and payer requirements
  • 7Follow professional standards and regulatory requirements
  • 8Participate in interdisciplinary team meetings to optimize resident outcomes
  • 9Contribute to the development of care plans aligned with goals

Requirements10

  • 1Current and active California Physical Therapist license
  • 2Graduate of an accredited Physical Therapy program
  • 3Must meet California requirements to practice as a Physical Therapist
  • 4Strong clinical, communication, and interpersonal skills
  • 5Ability to work effectively as part of an interdisciplinary team
  • 6Experience in skilled nursing or post-acute care preferred, new graduates welcome
  • 7onsite role in Los Angeles area
