Physical Therapist PT, Home Health San Diego & North Inland County
Overview
A licensed Physical Therapist will own in-home therapy services across the North Inland San Diego County region, delivering care per physician plans to restore mobility and independence. You will assess patients, tailor care plans, educate families, and collaborate with physicians and care teams to achieve meaningful outcomes. This role emphasizes high standards, continuous learning, and patient-centered service within Seaport Scripps Home Health family.
What You'll Do10
- 1Build and execute in-home therapy plans to improve mobility and independence
- 2Assess patients functional status and modify care plans for optimal outcomes
- 3Evaluate home environments and recommend safety modifications
- 4Deliver direct physical therapy treatments and educate patients and caregivers on techniques and equipment
- 5Monitor patient progress and adjust treatment plans accordingly
- 6Collaborate with physicians and interdisciplinary teams to coordinate care
- 7Document clinical notes and progress reports for successful discharge planning
- 8Supervise Physical Therapy Assistants per policy and regulations
- 9Participate in discharge planning and ensure smooth transitions to next care
- 10Maintain compliance with licensure and company policies
Requirements5
- 1Active California Physical Therapy License
- 2Minimum 2 years of physical therapy experience (home health preferred)
- 3Current CPR certification
- 4Valid driver’s license and insured, reliable vehicle
- 5Strong communication and organizational skills
