What You'll Do7
- 1Conduct comprehensive in‑home evaluations and design individualized plans for geriatric patients
- 2Implement therapeutic interventions focusing on functional strengthening gait training balance and fall risk reduction
- 3Schedule and manage home visits according to client availability and caseload demands
- 4Collaborate with multidisciplinary caregivers to coordinate optimal patient outcomes
- 5Utilize TherapySync EMR for efficient and compliant documentation
- 6Maintain professional licensure CPR BLS driver’s license auto insurance physical and TB test
- 7Obtain necessary professional liability and business licenses if required
Requirements6
- 15+ years experience providing skilled PT services in outpatient or SNF settings
- 2Certified in BCBA or related certification
- 3Driver’s license with auto insurance
- 4Physical examination clearance within the past year
- 5TB test result within the last twelve months
- 6Valid California physical therapist license
