Physical Therapist, Home Health Nevada & Field-Based Care

What You'll Do10
- 1Build and execute a clear plan of care for gait balance transfers and functional mobility
- 2Complete in home assessments and deliver skilled PT visits with measurable goals
- 3Coordinate with RN/MD and care team to adjust treatment and orders
- 4Document promptly to support defensible and efficient care
- 5Coach patients and caregivers with practical home programs that get implemented
- 6Supervise PTA where appropriate and manage treatment outcomes
- 7Route and schedule visits to maximize efficiency and patient access
- 8Maintain accurate documentation and progress tracking
- 9Ensure safety in mobility and home safety assessments
- 10Drive multidisciplinary collaboration to keep the care team aligned
Requirements5
- 1Active Nevada PT license in good standing
- 2Strong clinical judgment and independent field work ability
- 3Comfort escalating condition changes to RN/MD with rationale
- 4Excellent communication and documentation skills
- 5Reliable follow-through on orders, frequencies, and patient goals
