Occupational Therapist OT, Home Health Nevada & Field-based
Overview
This role owns in-home OT evaluations and skilled visits across Nevada at a field-based scale. You will build and execute a plan of care focused on ADLs IADLs, safety, fall prevention, and functional independence using adaptive equipment and home mods. You will coordinate with nursing and the care team, while our tech stack reduces documentation burden and supports scheduling to keep visits moving. You will have autonomy, meaningful patient impact, and a support team to remove administrative friction.
What You'll Do7
- 1Build and deliver in home OT visits and complete evaluations to inform care plans
- 2Execute a clear plan of care focused on ADLs IADLs safety and independence
- 3Train on adaptive equipment and home modifications as needed
- 4Coordinate with nursing and the broader care team when conditions change
- 5Document and communicate rationale for changes in condition to RN/MD and patient goals
- 6Contribute to the charting product by documenting visits efficiently and accurately
- 7Assist with scheduling and routing considerations to optimize daily caseload
Requirements8
- 1Active Nevada OT license in good standing
- 2Strong clinical judgment and ability to work independently in the field
- 3Ability to escalate changes in condition to RN/MD with clear rationale
- 4Strong communication and documentation skills
- 5Reliable follow-through on orders frequencies and patient goals
- 6Current driver’s license and ability to travel daily across assigned territory
- 7Ability to carry/transport basic therapy equipment
- 8Competency in core home health OT scope ADL IADL training home safety adaptive equipment caregiver education
