What You'll Do8

  • 1Implement treatment plans under the supervision of a Physical Therapist to improve patient mobility and function.
  • 2Document patient progress in PCC (EMR) with accuracy and timeliness.
  • 3Communicate effectively with therapy team, nurses, and physicians to coordinate care.
  • 4Adapt exercises and modalities to meet individual patient needs in a skilled nursing setting.
  • 5Educate patients and families on home exercise programs and safety strategies.
  • 6Participate in care conferences and contribute to discharge planning.
  • 7Maintain a clean and organized rehab gym, ensuring equipment is ready for use.
  • 8Track patient attendance and therapy minutes for billing and compliance.

Requirements7

  • 1Valid Physical Therapist Assistant license in Washington state.
  • 2CPR Certification required.
  • 3Proficiency in EMR documentation (experience with PCC preferred but not required).
  • 4Ability to work 15-20 hours per week consistently.
  • 5Compassionate and skilled in geriatric rehabilitation.
  • 6Strong communication and teamwork skills.
