You will own a caseload, collaborate with PT and nursing teams, and document care in Microsoft Outlook, Word, Excel, and electronic documentation systems. What You'll Do8
- 1Deliver PT services to residents in a sub-acute/skilled nursing setting, focusing on functional mobility and independence.
- 2Collaborate with rehab therapists and nursing staff to coordinate care plans and ensure consistent patient progress.
- 3Document patient evaluations, treatment notes, and progress in electronic documentation systems within required timelines.
- 4Adjust treatment approaches based on patient response and physician orders, maintaining high standards of care.
- 5Communicate with patients, families, and interdisciplinary team members to set goals and educate on home exercise programs.
- 6Manage your schedule efficiently to meet productivity expectations while preserving patient-centered care.
- 7Participate in team meetings and contribute to quality improvement initiatives in the rehab department.
- 8Mentor and support new staff when needed, sharing expertise in PT best practices.
Requirements7
- 1Hold a current Physical Therapy Assistant license or be license eligible in Massachusetts.
- 21+ years of experience in a skilled nursing or sub-acute setting preferred.
- 3Proficiency with Microsoft Outlook, Word, and Excel for communication and documentation.
- 4Experience with electronic documentation systems (e.g., PointClickCare, NetHealth) preferred.
- 5Strong attention to detail and ability to document accurately and on time.
- 6Demonstrated ability to work collaboratively in a team environment.
- 7Flexibility with per diem hours and ability to adapt to changing patient needs.
Salary Insight
