Overview
You will own patient treatment plans across the Atlanta area, delivering skilled care under the direction of a licensed Physical Therapist. Your caseload covers Hapeville, GA, and surrounding locations, with a Monday-Friday schedule. You will collaborate with a dedicated interdisciplinary team and keep Physical Therapists informed of patient progress. This role emphasizes patient education and home safety, setting you apart with direct community impact. As a PTA, you will implement care plans and adjust treatments within agency guidelines, aiming for measurable recovery outcomes.
What You'll Do8
- 1Provide skilled treatment per physician orders and agency policy, adjusting interventions to patient responses.
- 2Document each session with specific interventions, patient progress, and treatment priorities in the electronic health record.
- 3Educate patients and families on home safety and medical equipment use, reducing readmission risks.
- 4Communicate daily with supervising Physical Therapists, flagging any change in patient condition.
- 5Manage a caseload of 8-10 patients per day, meeting productivity targets while maintaining care quality.
- 6Attend weekly care conferences and contribute to interdisciplinary discharge planning.
- 7Coordinate with team members to ensure continuity of care across shifts and settings.
- 8Modify treatment plans based on patient tolerance and feedback, updating goals accordingly.
Requirements7
- 1Associates degree from an accredited Physical Therapy Assistant program.
- 2Valid state license to practice as a PTA in Georgia.
- 3CPR certification current and maintained throughout employment.
- 4Licensed driver with reliable transportation to patient homes.
- 51+ years of home health or outpatient experience preferred.
- 6Ability to lift and transfer patients up to 50 pounds.
- 7Electronic documentation proficiency with home health software.
